The third NF-104A (USAF 56-0762) was delivered to the USAF on 1 November 1963, and was destroyed in a crash while being piloted by Chuck Yeager on 10 December 1963.

Extremely nice work! I just wanted to add that this plane was also featured in the The Right Stuff. It is one of the best dramatizations of the actual events surrounding the early years of the American space program. It taught me the phrase 'screwed the pooch" when I first saw it around age 7 or 8. It's been years since I saw it last, but I think this specific plane is shown in detail both at the start and the very end of this epic classic film. Thanks for sharing!
